The Request for Tenure Clock Extension Form [MS Word] is to be submitted to the Office of the Provost for extenuating circumstances that warrant the need to extend a faculty member's tenure clock.

The Notification of Birth, Adoption, or Placement of a Foster Child Form is to be submitted by the faculty member to the Department Head. The Department Head is to send a copy of the form to Amy Jinnette, Campus Box 7526.
